SMS Terms of Service
By opting in to receive SMS messages from Affordable Care Health Clinic you agree to the following terms:
1. SMS Messaging Service
By providing your phone number, you consent to receive SMS messages, including appointment reminders, health updates, promotions, and other relevant two-way communication.
2. Message Frequency
You will receive a text message reminder one day prior to your scheduled appointment and up to 4 additional messages per year. You can always text us, and we will respond within 24 hours or less.
3. Message and Data Rates
Msg&data rates may apply based on your mobile carrier’s terms. Wireless carriers are not liable for delayed or undelivered messages.
4. Privacy Policy
Your information will be handled in accordance with our Privacy Policy, which can be viewed at https://www.achc-cares.com/privacy-policy/
5. Opt-Out Instructions
Opt-out at any time by replying “STOP” to any SMS message. Reply “HELP” for support. You also may contact us directly at [email protected] or 614-475-2273.
6. Liability
We are not responsible for any charges, errors, or delays in SMS delivery caused by your carrier or third-party service providers.
By opting in, you confirm that you are the owner or authorized user of the phone number provided and that you are at least 18 years old.
